Online Clinic
This online clinic is only for the use of our current patients. You must be attending a hospital-based diabetes clinic at either the Ards, Bangor or the Ulster Hospital to use this service. You can use the on-line clinic facility to contact your diabetes care team (consultant, diabetes nurse, dietitian or podiatrist) for advice or help.
So we know that you are one of our patients we ask for your name, hospital number and date-of-birth. You can find your hospital number on the top right hand corner of your green diabetes report sheet.
Once you have filled out all your personal information (name, hospital number, date-of-birth) complete the comment / question section and let us know what is bothering you and how we can help. Then decide how you want us to respond – by telephone, by e-mail or by a face-to-face meeting at your diabetes clinic. If you want a reply by e-mail then make sure we have your current e-mail address (in case we haven’t got the most up-to-date one.
As part of the on-line clinic we have designed a diary facility for you to add your most recent blood sugar results and some other measurements. This can be quite helpful to us if your query is about blood sugar control. There are instructions below on how to fill out the on-line glucose diary.
We plan to answer all on-line clinic queries within 2 working days.
Again please note: We can only respond to and give specific advice to patients who are registered with our service and who are currently attending hospital based diabetes clinics. We cannot extend this service to patients not currently attending hospital clinics or to patients outside the South Eastern Trust catchment area.
